Optimizing Care Transitions of Older Adults with Diabetes

Over 25% of adults over the age of 65 years have diabetes. We will discuss factors that contribute to complexity of care transitions for older adults with diabetes, interventions to support effective care transitions and challenges faced post discharge identified during interviews with patients and care-partners.
