This 30-minute session will provide an overview of lessons learned from the frontline by New York healthcare professionals in response to the COVID-19 pandemic including the importance of cross-setting communication and care coordination for infection control during care transitions.

This Learning & Action Network event is hosted by Alliant Quality, the quality improvement group of Alliant Health Solutions (AHS), the Medicare Quality Innovation Network-Quality Improvement Organization (QIN-QIO) for Alabama, Florida, Georgia, Kentucky, Louisiana, North Carolina, and Tennessee.
