Now that you have learned about diabetes by participating in Alliant Quality’s free diabetes classes as part of our Everyone with Diabetes Counts program, perhaps you are ready for the Next Step! Diabetes Self-management Education and Support (DSMES) programs can offer individual and group education.
- During an individual session, the diabetes educator can work with you to create a plan to fit your needs and lifestyle. It is personalized for you!
- In the group sessions, you will meet other people who have the same concerns and hear answers to their questions as well as yours.
- Ask your Primary Care Provider for a referral to a recognized DSMES program. A recognized program has been approved by the American Diabetes Association, the American Association of Diabetes Educators, or the Indian Health Services.
- Costs and copays may vary based on program location, income, and insurance coverage.
